Health Forms

BSA requires all Scouts and adults to have completed a medical form to participate in outings.

* For outings of 72 hours or less, our Troop must have a completed copy of Part A and C for each Scout and adult. Parts A and C can be completed by an adult.

* On outings that exceed 72 hours, the Troop needs to have a copy of Parts A, B and C. Part B must be completed by a doctor.
